Clinical Care Coordinator

Job Summary: We are seeking a skilled Clinical Care Coordinator to join our team at Premier Health. As a Clinical Care Coordinator, you will be responsible for coordinating patient care and ensuring seamless transitions between healthcare settings. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with interdisciplinary teams.
Key Responsibilities:
Develop and implement individualized care plans to promote optimal patient outcomes
Coordinate with healthcare providers to ensure timely and effective care transitions
Collaborate with patients, families, and caregivers to ensure informed decision-making and active participation in care planning
Utilize clinical knowledge and expertise to identify and address potential care gaps and barriers
Maintain accurate and up-to-date records of patient care and progress
Requirements:
Bachelor's degree in Nursing or related field
Minimum 3-5 years of experience in clinical care coordination or a related field
Valid RN license with the State of Ohio
Certification in case management or a related field preferred
