Job Summary

The RN Care Coordinator will work collaboratively with patients, family members, and healthcare providers to develop and implement personalized care plans that promote timely access to appropriate care, increase utilization of preventive care, and enhance patient health and well-being.

Key Responsibilities
  • Assess patient needs and develop care plans in collaboration with patients, family members, and healthcare providers.
  • Monitor patient progress, evaluate the effectiveness of care plans, and facilitate needed changes.
  • Provide education and support to patients and family members about relevant community resources and care coordination services.
  • Cultivate and support primary care and specialty provider co-management through timely communication, inquiry, follow-up, and integration of information into the care plan.
  • Identify high-risk patients and develop strategies to reduce emergency room utilization and hospital readmissions.
  • Document activities in the patient's electronic health record (EHR) and care plan.
  • Stay current on regulations, practices, and best practices in care coordination through ongoing training and education.
Requirements
  • Registered Nurse with current, unrestricted license in the state of Ohio.
  • Previous experience in community health center care coordination and/or case management experience preferred.
  • Current CPR certification.
  • Knowledge of ICD-10 and CPT coding preferred.
