Responsibilities- Provide admission, case management, and follow-up skilled nursing visits for home health patients.
- Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
- Collaborate with physicians to develop and implement individualized care plans, ensuring seamless coordination of care.
- Coordinate care with various healthcare professionals, including physical therapists, occupational therapists, and home health aides.
- Communicate effectively with patients, families, and healthcare teams to ensure optimal patient outcomes.
- Document patient care and progress accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Diploma, Associate, or Bachelor Degree in Nursing
- A minimum of one year of nursing experience, preferably in home health or a related setting
- Strong med-surg, ICU, ER, or acute care experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational and communication skills
- A valid driver's license, auto insurance, and reliable transportation
