About the Role

VNS Health is seeking a skilled Physical Therapist to join our team in providing high-quality care to patients in their homes and communities.

Key Responsibilities
  • Conduct comprehensive assessments of patients to determine their needs and develop personalized treatment plans.
  • Deliver rehabilitation therapy to patients in their homes or care facilities, focusing on improving their functional abilities and promoting independence.
  • Collaborate with interdisciplinary care teams to ensure seamless care coordination and patient outcomes.
  • Continuously evaluate patient needs and adjust treatment plans accordingly.
  • Minimum of one year of physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern.
  • Licensure to practice as a Physical Therapist in New York State.
  • Valid driver's license required for all areas outside the borough of Manhattan.
