About the Role:

This Bilingual Nurse Liaison position plays a critical role in our Care Management Department at Jamaica Hospital Medical Center. The successful candidate will work closely with patients, caregivers, and healthcare providers to ensure seamless transitions of care and effective management of chronic conditions.

Responsibilities:
  • Provide telephonic case management and transition of care services for patients recently discharged from the hospital
  • Collaborate with PCPs and other healthcare providers to develop and implement individualized care plans
  • Conduct thorough medical record reviews to assess patient health status and identify opportunities for improvement
  • Develop and deliver disease management education to patients with chronic conditions
  • Work effectively with patients, caregivers, and healthcare teams to ensure high-quality, patient-centered care
Qualifications:
  • Bachelor's degree in Nursing (BSN)
  • Current NY State RN License
  • Bilingual Cantonese/Mandarin speaking required
  • Minimum 2-5 years of inpatient nursing experience
  • Complex Case Management experience preferred
Compensation:

$65,000 - $85,000 per year, based on experience.
