Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our team at New Bridge Medical Center. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support to individuals and families in need.

Key Responsibilities
  • Conduct thorough screenings, intake assessments, and consultations to evaluate individual and family dynamics and treatment needs.
  • Provide ongoing social work intervention and education to patients, families, and significant others to address health-related psychosocial problems.
  • Utilize individual, family, group work, and crisis intervention methods to identify and intervene in problems that influence participation in treatment and discharge planning.
  • Complete comprehensive psychosocial histories in a timely manner.
  • Provide direct treatment services to patients within the hospital, utilizing appropriate modalities, including individual, family, and group counseling, and crisis intervention.
  • Serve as a resource to the healthcare team regarding issues of chemical dependence, psychiatric, and abuse issues, identifying, assessing, treating, and referring patients to community services as needed.
  • Provide case management services for assigned caseloads to ensure treatment plans are being followed and to aid patients in obtaining supportive services and resources.
  • Interpret the nature of illness and prescribed course of treatment to patients and families, communicating family issues to other staff members.
  • Act as a liaison to the healthcare team in understanding family dynamics, facilitating timely and proactive discharges to home or another facility.
  • Act as an advocate for the patient with other Medical Center programs and functions as a liaison with other providers in the community, assisting in linkages for patients and families.
  • Make presentations on various issues related to the field and on available Medical Center resources as part of community outreach to schools, agencies, and other resources.
  • Participate in interdisciplinary diagnostic and treatment planning conferences, including case presentations, and make recommendations regarding the needs of the patient from a social work perspective.
  • Fulfill all case recording, statistical reporting, and other documentation requirements in an accurate and timely manner.
  • Provide clinical supervision to paraprofessional staff and students as assigned.
  • Provide consultative services to the Department of Psychiatry and Medical Center staff and to other community agencies in the field regarding social work issues and services to our patient population.
  • Assist in developing discharge and post-hospital care plans, suggesting appropriate community resources.
Requirements
  • Masters Degree in Social Work from an accredited school of social work.
  • Licensed Social Worker (LSW) license preferred and must be obtained within one year from date of hire.
  • One to two years prior experience required in psychiatric social work or comparable experience/setting/population, depending on area assigned.
  • Three to five years experience preferred.
Preferred Qualifications
  • Demonstrated ability to work with psychiatrically impaired populations or those with addictions, as appropriate.
  • Good oral and written communication skills.
  • Good interpersonal skills.
  • Knowledge of a second language preferred.
