Key Responsibilities- Conduct comprehensive psycho-social assessments to identify patient needs and develop individualized care plans.
- Provide counseling, crisis intervention, and education to patients and their families, addressing health-related psycho-social problems.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and patient advocacy.
- Develop and implement discharge plans, ensuring patients receive necessary support and resources for a smooth transition.
- Participate in inter-disciplinary care planning conferences, contributing social work expertise to patient care.
- Masters Degree in Social Work from an accredited school of social work.
- Valid NJ Licensed Clinical Social Worker (LCSW) license.
- 1-2 years of experience in a healthcare setting, preferably with geriatric populations.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks.
