We are seeking a highly skilled and compassionate Social Worker to join our team at New Bridge Medical Center. As a key member of our healthcare team, you will provide essential social work services to patients and their families, ensuring they receive the support and care they need during their hospital stay.
Key Responsibilities- Conduct thorough psychosocial assessments to identify patients' needs and develop individualized treatment plans.
- Provide counseling and education to patients and their families on health-related psychosocial issues, promoting healthy coping mechanisms and support systems.
- Collaborate with healthcare teams to ensure seamless care coordination and discharge planning.
- Identify and address potential barriers to treatment, such as chemical dependence, psychiatric issues, or abuse.
- Develop and maintain relationships with community agencies and resources to facilitate post-hospital care and support.
- Participate in interdisciplinary diagnostic and treatment planning conferences to ensure comprehensive patient care.
- Maintain accurate and timely documentation of patient interactions, treatment plans, and outcomes.
- Masters Degree in Social Work from an accredited school of social work.
- Licensed Social Worker (LSW) license preferred and must be obtained within one year from date of hire.
- One to two years prior experience required in psychiatric social work or comparable experience/setting/population.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Knowledge of a second language is preferred.
