Health Promotion Center Nurse Practitioner

Job Summary:
We are seeking a highly skilled and compassionate Nurse Practitioner to join our Health Promotion Center team. As a key member of our team, you will work closely with other healthcare professionals to provide high-quality patient care and promote health and wellness in our community.
Responsibilities:
- Collaborate with the Health Promotion Center team to develop and implement patient care plans.
- Conduct patient visits and provide comprehensive health assessments.
- Develop and maintain accurate and up-to-date patient records.
- Consult with supervising physicians and other healthcare professionals as needed.
- Assist patients with Project Access Applications and Management.
Qualifications:
- Masters or Doctoral prepared Nurse Practitioner.
- NP CA license, NP CA Furnishing License, NPI.
- National NP Certification (ANCC or AANP).
- CPR Certified (BLS required).
Physical Requirements:
- The work is primarily sedentary and may require sitting for extended periods of time.
- Prolonged periods of standing during patient evaluations.
