About the Role:We are a non-profit, multi-site community health organization dedicated to providing comprehensive health services to the Mission and Excelsior Districts of San Francisco. As a Primary Care Nurse Practitioner, you will be responsible for providing high-quality, patient-centered care to our diverse patient population.
Key Responsibilities:- Provide ambulatory acute, chronic, and preventative medical services to patients in accordance with Community and National Standards of Practice and DHHS/DHCS guidelines.
- Provide continuity of care for hospitalized and referred patients according to Health Center protocols.
- Lead and guide support staff, volunteers, and trainees in day-to-day clinic functions.
- Adhere to the highest standards of medical practice, ethics, and professionalism at all times.
- Support process improvement initiatives that enhance Team Based Care and PCMH recognition.
- Current RN/NP License and Furnishing Number issued by the California Board of Registered Nursing.
- Master's degree in Nursing and/or Doctorate in Nursing Practice.
- National certification as a Family Nurse Practitioner or an Adult Nurse Practitioner from the American Academy of Nurse Practitioners, American Nurses Credentialing Center, or other national NP certifying body.
- At least 18 months experience as Nurse Practitioner preferred, but not required.
- Spanish fluency is helpful, but not required.
- Competitive salary range of $130,400 to $143,500.
