Job Summary

We are seeking a highly skilled Full-time Registered Nurse Case Manager to join our team in the greater Philadelphia area (Northeast Philadelphia and surrounding region). This role offers a $10K Sign On Bonus and comes with a competitive salary, benefits, and opportunities for professional development.

Responsibilities

As a Full-time Registered Nurse Case Manager, you will provide primary nursing care to home-based care patients as ordered by the physician, and in accordance with the organization's policies and procedures. Your responsibilities will include:

  • Initial assessment and care plan development in collaboration with the Population Health Case Manager
  • Providing functional support/supervision to Home Healthcare Aides and LPNs as needed
  • Providing therapeutic intervention, overall care management, and achievement of top decile client outcomes and patient satisfaction
Requirements

To be successful in this role, you will need to have:

  • Graduate of an approved nursing education program
  • Licensure as a Registered Nurse in the state of PA
  • One (1) year experience as a professional care nurse
  • Current Driver's license and reliable transportation

The estimated annual salary for this role is around $85,000-$110,000, depending on experience and qualifications.
