Job Summary

We are seeking a highly organized and detail-oriented LPN Coordinator to join our team at Amedisys. In this role, you will be responsible for scheduling home visits for our field staff and assisting with the coordination of patient care.

Key Responsibilities
  • Schedule patient assessment visits, including Oasis visits, in a timely manner
  • Prepare weekly clinician schedules and review them daily to ensure productivity standards are met
  • Ensure clinicians are assigned and scheduled in the most efficient geography, maximizing clinician efficiency
  • Manage calls from patients and field staff related to scheduling issues
  • Collaborate with field clinicians to ensure all patient visits are scheduled and completed as ordered
  • Monitor hospitalized patients and communicate with the team as necessary
  • Complete requested schedules, including lab draws, supervisory visits, and add-on visits
  • Support the Clinical Manager and communicate with patients, clinicians, referral sources, and other office personnel
  • Run reports and respond to workflow, taking appropriate actions
  • Assist with internal or external transfers of patients between care centers and/or Hospice services
Requirements
  • Current, unencumbered license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse or Registered Nurse
  • High School diploma or GED equivalent
  • One year of administrative experience in a healthcare environment
  • Preferred: one year of scheduling experience in a healthcare environment
