As a Nurse Practitioner at Trinity Health, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.
Key Responsibilities:
• Conduct clinical assessments of patients' healthcare status and needs
• Provide patient care to facilitate expected patient outcomes
• Assist physicians with follow-up patient visits, intra-procedure, hospital consults, and other duties within scope of practice
• Provide counseling and education to patients to enhance patient care
• Document care provided to support the continuity of patient care and optimum patient outcomes
Requirements:
• Graduation from an accredited school for a Registered Nurse Practitioner program
• Master's Degree in Nursing
• Minimum of 1 year post-licensure clinical nursing experience
• Privileges and/or eligibility to apply are necessary
