Job Summary

The Community Case Manager position at Veterans Place is a critical role in supporting the transition of homeless veterans into permanent housing. As a key member of our team, you will provide in-home visits, support, and guidance to veterans, helping them set and track personal goals, sustain a healthy lifestyle, and progress toward independence and housing retention.

Key Responsibilities
  • Conduct assessments and develop individualized service plans (ISPs) for veterans, incorporating housing retention, employment, and financial goals.
  • Provide in-home visits, meetings, and referrals as needed, facilitating housing search and coordination of resources.
  • Collaborate with the Veterans Administration and other community providers to ensure a comprehensive support network for veterans.
  • Develop and implement grant-related group weekly sessions to promote housing retention and stability.
  • Prepare reports and documents as requested, communicating effectively with Veterans Place and Veterans Administration staff.
Requirements
  • Bachelor's degree in a social service field or equivalent experience.
  • Proficiency in Microsoft Office Suite, including Word, Excel, and PowerPoint.
  • Experience working with homeless populations and the Veterans Administration is preferred.
  • Valid driver's license and ability to obtain automotive insurance.
  • Veteran status is a plus.
