Key Responsibilities- Assess and Plan Patient Care: Conduct thorough assessments of patients' medical histories, current medications, and treatment plans to identify potential medication-related problems and develop effective treatment plans.
- Order and Evaluate Laboratory Tests: Order and evaluate laboratory tests to monitor patients' response to medication therapy and adjust treatment plans as needed.
- Initiate, Modify, or Discontinue Medications: Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states, in cooperation with the medical staff.
- Monitor and Manage Pharmacotherapy: Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics.
- Administer Medications: Administer prescription or non-prescription drugs according to established agreements or protocols.
- Evaluate Medical and Medication Histories: Evaluate medical and medication histories for drug-related problems and adjust treatment plans accordingly.
- Consult with Healthcare Providers: Consult with other healthcare providers about patient treatment needs or options.
- Perform Pharmacist Procedures: Perform a full range of pharmacist procedures, including reviewing patient profiles at the time of dispensing.
- Ensure Prescription Accuracy: Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
- Provide In-Service Training: Provide medication and healthcare-related in-service training to clinical staff on new medications, appropriate use of medications, and other topics as deemed appropriate.
- Document Significant Interactions: Document significant medication interactions and pharmacy interactions with prescribers.
- Conduct Adverse Drug Reaction Reporting: Conduct Adverse Drug Reaction (ADR) reporting in accordance with The Joint Commission or the Accreditation Association for Ambulatory Health Care, Inc. standards.
