LPN - SOVAH Heart & Vascular Clinic

Job Summary:
- Provide clinical support to patients in the outpatient clinic under the supervision of the physician.
- Assist with patient care, including vital sign collection, medication administration, and laboratory testing.
- Collaborate with the healthcare team to ensure seamless patient care.
- Participate in quality improvement initiatives to enhance patient outcomes.
Responsibilities:
- Triages patients by phone and as walk-ins to the clinic.
- Escorts patients to exam rooms and monitors patient flow.
- Accesses computer systems for patient reports and prepares patient charts with pertinent medical information.
- Obtains and records patient vital signs and medication lists.
- Performs office procedures and assists providers with minor clinical procedures.
- Manages prescriptions and calls in/faxes and eRx prescriptions as directed by the provider.
- Schedules diagnostic tests and procedures as ordered by the provider.
- Obtains patient referrals, pre-authorizations, and pre-certifications using eCW.
- Performs various administrative duties, including answering phone calls, returning calls, receiving and sorting incoming faxes, and completing forms as requested.
- Monitors, documents, and communicates patient status and reactions to interventions with the RN, physician, patient, and families.
- May administer medications, injections, and immunizations, and utilizes office and lab equipment for inpatient orders and lab tests.
Requirements:
- Current LPN license in the Commonwealth of Virginia or compact State required.
- Previous LPN experience preferred.
- Requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision.
- Basic Life Support (BLS) American Heart Association (AHA) required.
