Job Title: Private Duty Nurse LPN

Job Summary:

Aveanna Healthcare is seeking a skilled and compassionate Private Duty Nurse LPN to provide high-quality patient care in the comfort of their own homes. As a Private Duty Nurse LPN, you will work closely with patients and their families to develop and implement personalized care plans that meet their unique needs and goals.

Responsibilities:

  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Develop and implement individualized care plans that address the patient's physical, emotional, and social needs.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Conduct thorough assessments of patients' conditions and develop plans to address any identified needs or concerns.
  • Administer medications and treatments as prescribed by physicians, and document all care provided.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Participate in ongoing education and training to stay current on best practices and industry developments.

Requirements:

  • Current LPN licensure in the state of employment.
  • 6 months of experience as a practicing nurse, with a focus on home health care or pediatric nursing.
  • CPR certification and ability to work in a fast-paced, dynamic environment.
