We are seeking a dedicated and compassionate Private Duty Nurse LPN to join our team of healthcare professionals. As a Private Duty Nurse LPN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role

This is a full-time position that offers a competitive salary and benefits package. As a Private Duty Nurse LPN, you will be responsible for providing high-quality patient care in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
  • Document all patient interactions and care provided in a timely and accurate manner.
  • Participate in ongoing education and training to stay up-to-date on the latest best practices and technologies in home health care.
Requirements
  • Current LPN licensure in the state of employment.
  • At least 6 months of experience as a practicing nurse.
  • CPR certification.
