We are seeking a dedicated RN Care Manager to join our Continuum of Care Department at Parkwest Medical Center. As a RN Care Manager, you will play a crucial role in promoting patient-centered care and ensuring that patients receive the highest quality care during their stay and after discharge.

Estimated Salary: $72,000 - $90,000 per year

Job Summary:

The RN Care Manager will be responsible for coordinating patient care, developing care plans, and communicating with healthcare providers to ensure timely and effective care. This role requires strong organizational and time management skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:

  1. Assess patients' medical, emotional, and social needs
  2. Develop and implement comprehensive care plans
  3. Coordinate with healthcare providers, including physicians, nurses, and social workers
  4. Ensure timely and effective care transitions
  5. Monitor and evaluate care plans to ensure optimal outcomes

Requirements:

  • Bachelor's degree in Nursing (BSN) required
  • Current Tennessee RN license required
  • Minimum 3 years of nursing experience, preferably in acute care setting
  • Strong organizational and time management skills
