Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Float Nurse. In this role, you will provide high-quality patient care and support to our patients and staff.

Key Responsibilities
  • Assist healthcare providers with patient office visits, including rooming patients, interviewing patients, and recording vital signs.
  • Perform point-of-care testing and quality controls, including urinalysis, pregnancy testing, blood glucose monitoring, HgA1c, and Hematocrit.
  • Obtain and verify informed consent from patients, parents, or guardians, and provide clear explanations and encouragement as necessary.
  • Administer and document medications under physician's orders, following the 6 medication rights to reduce the potential for medical errors.
  • Schedule appointments for immediate patient needs, track hospital admissions and discharges, and provide appointments from hospital discharges.
  • Follow up on patients who did not keep their appointments, diagnostic testing, or lab appointments, or follow through on referrals.
  • Communicate with outside resources to assist patients with medications and to self-educate about new medications for teaching purposes.
  • May answer phone calls and relay patient information to healthcare providers, and have the ability to manage a patient call and collect data to relay to the RN or provider.
  • May obtain prior authorizations from insurance companies for medication and other services ordered by providers.
  • Understand the importance of quality measure guidelines and take the initiative to have patients fill out ROI to obtain pertinent medical records.
  • Have basic knowledge of adult and children immunization schedules and be able to schedule and give age-appropriate immunizations.
  • Work assigned clinical inbox daily and remain within assigned threshold parameters.
  • May be responsible for training new hires/students.
  • Initiate referrals to community agencies as directed by healthcare providers and offer patient resources to assist with daily needs, shelter, food, transportation, and medical education.
  • Assist other nursing staff with organizing and stocking supplies and medications, and contribute to the orderliness and cleanliness of the patients' and staffs' environment.
Requirements
  • Graduate of an accredited program for LPN and hold a current license to practice in the State of Illinois.
  • Current CPR certification.
  • Ambulatory care experience preferred.
Work Environment

This is a light physical activity position performing non-strenuous daily activities of a clinical nature. Must be able to lift up to 50 pounds.

Work Schedule

Normal work hours for this position are Monday through Friday dayshift, unless otherwise specified by management staff.
