About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our Palliative Care team at Illumia. As a Palliative Care RN, you will play a vital role in providing high-quality end-of-life care to patients and their families.

Key Responsibilities
  • Conduct in-person and telephonic assessments to identify patient support needs and develop personalized care plans.
  • Collaborate with the interdisciplinary team to provide comprehensive care, including education, symptom management, and emotional support.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Document timely and accurate clinical records, including screening/assessment findings, physician orders, progress notes, and care plans.
  • Participate in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.
Requirements
  • 2 years of prior skilled Registered Nurse experience.
  • Demonstrated ability to provide quality patient care.
  • Nursing experience in palliative care, oncology, hospice, or geriatric care preferred.
  • Current RN license to practice in the state and CPR Certification.
