Palliative Care Nurse

We are seeking a highly skilled and compassionate Registered Nurse to join our Palliative Care team at Illumia. As a Palliative Care Nurse, you will play a vital role in providing high-quality end-of-life care to patients and their families.
Key Responsibilities- Conduct in-person and telephonic assessments to identify patient support needs and develop personalized care plans.
- Collaborate with the interdisciplinary team to provide education and support to patients and families.
- Coordinate care with palliative team members to ensure seamless delivery of services.
- Document timely clinical records and communicate findings to the Palliative NP and/or PCP.
- Support hospice referrals as needed and facilitate in-person visits to patients' homes.
- 2+ years of prior skilled Registered Nurse experience.
- Demonstrated ability to provide quality patient care and work effectively in a team environment.
- Nursing experience in palliative care, oncology, hospice, or geriatric care preferred.
