About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our Palliative Care team at Illumia. As a Palliative Care Nurse, you will play a vital role in providing high-quality end-of-life care to patients and their families.

Key Responsibilities
  • Conduct in-person and telephonic assessments to identify patient support needs and develop personalized care plans.
  • Collaborate with the interdisciplinary team to provide education and support to patients and families.
  • Coordinate care with palliative team members to ensure seamless delivery of services.
  • Document timely clinical records and communicate findings to the Palliative NP and/or PCP.
  • Support hospice referrals as appropriate and facilitate in-person visits as needed.
Requirements
  • 2 years of prior skilled Registered Nurse experience.
  • Demonstrated ability to provide quality patient care.
  • Nursing experience in palliative care, oncology, hospice, or geriatric care preferred.
