Job Summary

We are seeking a highly skilled and compassionate Clinical Practice Nurse to join our team at Concord Hospital, Inc. As a key member of our healthcare team, you will provide high-quality patient care, working closely with our clinical leaders and providers to deliver exceptional outcomes.

Key Responsibilities
  • Assess patient needs and develop individualized care plans
  • Administer medications and provide education to patients and families
  • Collaborate with healthcare teams to ensure seamless patient care
  • Document patient information and outcomes in electronic medical records
  • Delegate patient care activities to ensure efficient and effective care
Requirements
  • Associate Degree or Diploma in Nursing from an accredited School of Nursing
  • New Hampshire Board of Nursing license
  • American Heart Association Basic Life Support for Healthcare Providers certification or Red Cross CPR/AED for Professional Rescuers and Health Care Providers
Work Environment
