About the Role

Pinnacle Home Care Inc. is seeking a highly skilled and compassionate Physical Therapist to join our team of dedicated healthcare professionals. As a Physical Therapist with our company, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities
  • Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Develop and implement personalized treatment plans in consultation with physicians and revise as necessary.
  • Assist in the development and implementation of interdisciplinary client care plans as it pertains to Physical Therapy.
  • Treat patients to relieve pain, develop or restore functional abilities, and maintain maximum performance using physical means such as exercise, massage, heat, light, and electricity.
  • Teach, supervise, and counsel patients and their families in the physical therapy program, instructing them in the care and use of adaptive devices such as wheelchairs, braces, crutches, canes, and prosthetic and orthotic devices.
  • Record all evaluation data, treatments, and patients' responses to therapy intervention in a timely manner, as well as quality assurance data as directed.
  • Prepare evaluations of patients' conditions and perform periodic reassessments.
  • Supervise Physical Therapist Assistants at least every 30 days.
  • Participate in case conferences, physical therapy staff meetings, agency activities, and in-service programs as necessary.
  • Participate with staff, patients, and physicians in discharge planning activities and complete a Physical Therapy Discharge Summary.
Requirements
  • A graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
  • Licensed or registered by the State.
  • At least one year of institutional experience.
  • A Physical Therapist in charge of PT services must have a home care experience and at least one year of supervisory experience (2 years preferred).
  • One year of experience in an acute/sub-acute setting.
