Pinnacle Home Care Inc. is seeking a skilled Physical Therapist to join our team of dedicated healthcare professionals. As a Physical Therapist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' conditions, including Outcome and Assessment Information Set (OASIS) assessments.
  • Develop and implement personalized treatment plans in consultation with physicians and revise them as necessary.
  • Collaborate with the interdisciplinary team to develop and implement client care plans.
  • Provide physical therapy services to patients, including exercises, massage, heat, light, and electricity treatments.
  • Teach and supervise patients and their families on proper use of adaptive devices, such as wheelchairs, braces, and prosthetic and orthotic devices.
  • Record and report patient progress, including evaluation data, treatments, and client response to therapy intervention.
  • Participate in case conferences, staff meetings, and in-service programs to ensure high-quality care and patient safety.
Requirements:
  • Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
  • Licensed or registered by the State.
  • At least one year of institutional experience.
  • Home care experience and at least one year of supervisory experience (2 years preferred).
  • Ability to drive and access clients' homes, which may not be routinely wheelchair accessible.
  • Excellent communication and interpersonal skills.
