About the Role

We are seeking a compassionate and skilled Spiritual Care Coordinator to join our team at Agape Care Group. As a Spiritual Care Coordinator, you will play a vital role in providing spiritual support and guidance to patients and their families, honoring their beliefs and values.

Key Responsibilities
  • Provide spiritual support and guidance to patients and their families, respecting their individual beliefs and values.
  • Collaborate with healthcare teams to ensure comprehensive care that addresses the physical, emotional, and spiritual needs of patients.
  • Develop and implement spiritual care plans that promote patient well-being and comfort.
  • Offer counseling and support to patients and families dealing with loss, grief, and end-of-life issues.
  • Participate in interdisciplinary teams to ensure seamless care coordination and communication.
Requirements
  • Bachelor's degree in counseling, theology, divinity, or a related field.
  • 2+ years of experience in a healthcare or hospice setting, with a focus on spiritual care and counseling.
  • Reliable transportation and ability to sit, stand, bend, move intermittently, and lift at least 25 pounds.
