Position InformationThe Care Coordinator RN is a key member of our Financial Clearance Department, responsible for facilitating the physician care plan for patients during their hospital encounter.
Using a collaborative interdisciplinary approach, the Care Coordinator will confirm the plan of care and focus on anticipating, integrating, and coordinating clinical care to meet the needs of patients, optimizing clinical and resource outcomes.
The Care Coordinator will work closely with Physician offices, Physician Advisors, Point of Entry Case Managers, Unit-Based Case Managers, and Case Management leadership to assess patient needs and develop the plan of care.
Additionally, the Care Coordinator will act as an advocate for patients, physicians, and the hospital, ensuring compliance with federal, state, and third-party utilization management regulatory requirements.
Experience Requirements- 3 years of experience in nursing, with a focus on case management or utilization management
- Graduate nursing degree required; Bachelor's degree preferred
- Registered Nurse licensure required; Certified Case Manager or Commission for Case Management Certification preferred
Monday to Thursday, 9:00am - 6:00pm; no weekends or call requirements
