Job Summary

The RN Care Manager will provide ongoing support and expertise through comprehensive assessment, planning, implementation, and evaluation of individual patient needs. The overall goal of this position is to enhance the quality of patient management and satisfaction, promote continuity of care, and cost-effectiveness through the integrating functions of case management, utilization review, and discharge planning.

Key Responsibilities

Identify cases and prioritize day-to-day activities to ensure timely and effective patient care.
Conduct and document assessments and plans of care in EpicTM per departmental guidelines.
Participate in daily care management touchpoints and consult with social workers as needed.
Attend and actively participate in CAPP meetings to provide and receive information on patients' progression and modify discharge plans accordingly.
Collaborate with the utilization manager and social worker to problem-solve issues with complex patients and identify trends.
Discuss barriers to discharge and psychosocial concerns with the multidisciplinary team and coordinate family meetings as necessary.
Communicate medical milestones for transition with patients and families and identify patients' readiness to discharge based on ongoing discussions with the care team.

Requirements

Graduation from a state-accredited school of professional nursing.
Licensed to practice as a Registered Nurse in the state of North Carolina.
Two years of healthcare experience as a Registered Nurse.
Strong assessment and critical thinking skills.

Work Environment

This position is employed by UNC Health (Rex Healthcare, Inc., d/b/a NC Health), a private, fully-owned subsidiary of UNC Health Care System. The work environment is dynamic, and the RN Care Manager must be flexible and adaptable to frequent changes in priorities and assignments. The position requires strong critical thinking skills to evaluate and prioritize rapidly changing demands and work collaboratively to best accomplish the team's mission.
