Position Information:

The Care Coordinator RN is a key member of our interdisciplinary team, responsible for facilitating the plan for inpatient and outpatient care. This role requires a collaborative approach, working with patients, physicians, and other healthcare professionals to ensure seamless care coordination and optimal clinical outcomes.

Key Responsibilities:

• Anticipate and integrate clinical care and discharge planning services to meet patient needs

• Collaborate with the interdisciplinary team to assess patient needs and develop the plan of care

• Act as an advocate for patients, physicians, and the hospital, ensuring compliance with federal, state, and third-party utilization management regulatory requirements

Requirements:

• 3 years of experience in a nursing-related area, with a focus on clinical case management, utilization management, or home or community health

• Graduate nursing degree required, with a bachelor's degree in nursing preferred

• Registered Nurse licensure required

Work Schedule:

Monday, Tuesday, and Wednesday, 2pm to 10pm

Weekend and Call Requirements:

No weekends, no call requirements
