Job Summary

We are seeking a compassionate and patient Caregiver - Mentor and Support Specialist to join our team. As a Direct Support Professional, you will work with individuals with intellectual and developmental disabilities, providing support and guidance to help them achieve their goals and live fulfilling lives.

Key Responsibilities
  • Provide personal care and support to individuals with complex needs, including assistance with daily living activities and behavioral challenges.
  • Develop and implement individualized support plans to help participants achieve their goals and improve their quality of life.
  • Build and maintain positive relationships with participants, their families, and other stakeholders to ensure a collaborative and supportive environment.
  • Collaborate with other team members to provide comprehensive support services and ensure a seamless delivery of care.
  • Document participant progress and provide regular updates to supervisors and other stakeholders.
Requirements
  • High School Diploma or equivalent required.
  • 6 months of experience in human services, direct care, or care coordination preferred.
  • Valid Driver's license and access to a registered vehicle with proof of insurance.
  • Excellent communication, multi-tasking, and adaptability skills.
  • Patient, compassionate, reliable, and responsible behavior.
