Key Responsibilities

The Patient Care Coordinator is accountable for overseeing the quality and resource management of all patients admitted to the facility, from the moment of admission through the entire continuum of healthcare management. This role advocates for patients, emphasizing cost-effectiveness while demonstrating leadership to integrate healthcare providers for a seamless care delivery experience. The approach is designed to ensure the attainment of quality, clinical, and cost-efficient outcomes, while conducting thorough and comprehensive reviews of medical records for medical necessity, service intensity, and illness severity.

Qualifications

Education and Experience Requirements:

  • Minimum of 5 years of professional experience following graduation from an accredited nursing program, along with a current state Registered Nurse license.
  • For those grandfathered prior to a specified date, a minimum of 5 years post-graduation from an accredited School of Social Work for Licensed Clinical Social Workers is acceptable; however, a Registered Nurse Case Manager is preferred.
  • Preferred experience includes five years in acute care nursing and at least one year in case management, discharge planning, or nursing management.
  • Current Basic Life Support (BCLS) certification from the American Heart Association is required upon hire and must be maintained.
  • Familiarity with Milliman and InterQual Criteria is preferred.
  • Proficiency in basic to intermediate computer skills is necessary.
  • Completion of Behavioral Violence Prevention Training within three months of hire and maintenance of current certification is required.
