Job Summary

We are seeking a highly skilled Clinical Social Worker to join our team at Spaulding Rehabilitation. As a key member of our multidisciplinary team, you will provide comprehensive social work services to patients and families, addressing their psychosocial needs and facilitating their adjustment and coping skills.

Key Responsibilities
  • Conduct thorough clinical social work assessments to identify patient and family needs and develop individualized plans of care.
  • Collaborate with the healthcare team to develop and implement treatment plans that address the physical, emotional, and social needs of patients.
  • Provide education and support to patients and families regarding healthcare and community resources.
  • Facilitate patient and family adjustment and coping skills through effective counseling and support.
  • Develop and maintain relationships with community and social service agencies to access resources that benefit patients and families.
  • Provide professional consultation and education to healthcare professionals and students to advance best practices in social work.
  • Document patient interactions and progress according to department and regulatory standards.
  • Identify and discuss performance improvement opportunities with stakeholders and implement action plans as needed.
  • Collaborate with program leadership to design and implement programs that achieve optimal patient outcomes.
Qualifications
  • Masters degree in Clinical Social Work from an accredited school of social work.
  • One to two years of clinical experience in a medical setting with a mental health background preferred.
  • Current Massachusetts license as LICSW.
Skills
  • Computer proficiency with Microsoft Office applications.
  • Excellent communication and interpersonal skills with diverse populations.
  • Ability to work independently and as a team member.
  • High level of service delivery with attention to detail.
