Mass General Brigham is seeking a skilled Social Worker to provide per diem services in East Sandwich, Massachusetts.

As a Social Worker at Mass General Brigham, you will assess patients and families, providing intervention and treatment to support their well-being. You will address psychosocial needs and patterns of adjustment throughout the hospitalization, collaborating with the multidisciplinary team to formulate treatment plans that maximize emotional, social, and physical well-being.

Key responsibilities include:

  • Completing clinical social work assessments within established timeframes and in accordance with the Clinical Social Work Scope of Practice guidelines.
  • Developing plans of care and interventions that meet patient and family psychosocial needs, consistent with individual goals and environmental context.
  • Facilitating patient and family adjustment and coping skills through effective counseling.
  • Providing community and social service agency relationships to access community resources that benefit patients and families.
  • Teaching and mentoring new staff and healthcare students to achieve identified performance expectations.
  • Delivering educational in-services and case conference presentations to integrate best practices into service.
  • Providing professional consultation to healthcare professionals and hospital personnel to effectively manage complex patient/family or interpersonal situations.
  • Completing assessments of suspected abuse, neglect, and domestic violence, and filing reports in compliance with regulatory requirements.
  • Documenting according to department, hospital, and regulatory standards.
  • Identifying performance improvement opportunities, discussing with stakeholders, creating and implementing action plans as appropriate.
  • Facilitating care and progression toward discharge by effective alignment with hospital length of stay and financial reimbursement goals and initiatives.
  • Completing documentation to support third-party payer requirements.
  • Collaborating with key program leadership to improve program design, structure, and resources to achieve optimal patient outcomes.
  • Conducting research, supported or applied to the workplace to advance best practices.
  • Completing additional department, organization, or network activities per established objectives.

Requirements include:

  • Masters degree in Clinical Social Work from an accredited school of social work.
  • One to two years of clinical experience in a medical setting with a mental health background preferred.
  • Current Massachusetts license as LICSW.
  • Computer proficiency required, with Microsoft office applications preferred and ability to learn new software.
  • Highly developed communications and interpersonal skills, working with diverse populations.
  • Ability to work independently, be self-directed, and contribute as a member of a team.
  • Anticipating challenges and developing strategies for addressing them.
  • High level of service delivery, with attention to detail.
