LSS of WI & Upper MI is a leading organization dedicated to supporting seniors and individuals with disabilities. As an Onsite Service Coordinator, you will play a vital role in promoting self-sufficiency and independence within our residential communities.

Job Summary

We are seeking an experienced Service Coordinator to provide supportive services to our tenants in Sparta and Richland Center. The successful candidate will work closely with the Program Supervisor to deliver community connections, presentations, and recreational programming.

About the Role
  • As an Onsite Service Coordinator, you will connect residents with local resources, facilitate workshops, and promote tenant engagement.
  • You will assess health, psychological, and social needs, developing tailored case plans and reassessing situations periodically.
  • The ideal candidate will establish linkages with community agencies, negotiate service pricing, and maintain directories of providers.
  • You will refer tenants to service providers, educate them on available services, and advocate as necessary.
Requirements
  • A Bachelor's degree in Social Work, Sociology, or a related field, or equivalent experience.
  • At least 3 years' experience working with older adults, preferably in a supportive services capacity.
  • A valid driver's license, reliable transportation, and a satisfactory driving record.
  • Excellent communication, documentation, and interpersonal skills.
