Rehabilitative Care Specialist

As a Rehabilitative Care Specialist, your primary responsibility is to deliver care to residents at the highest standard. You will be assigned to multiple facilities, and frequent travel may be required.

You will play a vital role as part of the interdisciplinary team, managing resources to meet the needs of residents and related customers while balancing outcomes with cost-effective and efficient treatment.

Essential Duties and Responsibilities:


• Provide and direct resident rehabilitation care in accordance with all generally accepted practice methods, ensuring compliance with center, state, federal, and accrediting standards.
• Assess resident needs and develop effective and efficient treatment plans for assigned residents.
• Oversee and coordinate the supervision of treatment plans with all staff and other disciplines as necessary.
• Participate in the screening of all residents as per directed schedules and practices.
• Provide feedback to the designated supervisor on all clinical and rehabilitation matters.
• Engage in all center-directed processes and services.
• Demonstrate effective management of treatment and non-treatment related activities.
• Complete necessary clinical documentation to meet center, state, federal, and accrediting agency requirements.
• Maintain accurate and timely billing records to comply with center, state, federal, and accrediting agency standards.
• Assist with marketing efforts as directed.
• Participate in in-service training programs as requested.
• May travel to other facilities to provide services.


Qualifications:

Position Requirements:
• Master's Degree in Physical Therapy
• Current and Active Physical Therapist license within the state of operations
• Preferred experience/knowledge in older adult rehabilitation.
• Preferred knowledge of governmental and regulatory guidelines in designated healthcare settings.
