Overview

Pinnacle Home Care Inc. is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Nurse. As a Home Health Nurse, you will play a vital role in delivering high-quality patient care and improving the lives of our patients.

Responsibilities

  • Conduct thorough assessments of patients to determine their home care needs and develop personalized care plans.
  • Provide a range of nursing services, including medication management, pain control, and wound care.
  • Collaborate with patients and their families to ensure seamless care transitions and address any concerns or questions they may have.
  • Monitor patient progress and make adjustments to care plans as needed.
  • Communicate effectively with healthcare teams, including physicians, therapists, and other healthcare professionals, to ensure comprehensive care.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.

Requirements

  • Associate's or bachelor's degree in nursing with at least one to two years of recent acute care experience in an institutional setting.
  • Two years of home health experience preferred.
  • Current licensure as a Registered Nurse (RN) in the state of practice.
  • Current CPR certification.
  • Licensed driver with a reliable vehicle and valid insurance.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
