POSITION SUMMARY
The Practical Nursing Specialist is tasked with delivering essential care and services to designated patients under the guidance of a Registered Nurse, adhering to the individualized care plan established for each patient.
KEY RESPONSIBILITIES- Administers care and services to patients, including the delivery of treatments and medications as outlined in the patient's care plan.
- Documents clinical observations and progress notes accurately and promptly.
- Maintains comprehensive and timely records of the care and services rendered.
- Alerts the Registered Nurse to any alterations in the patient's condition or care plan.
COLLABORATIVE CARE
Engages in the nursing process by assessing and suggesting modifications to the care plan as necessary.
- Conducts treatments or specialized procedures only as authorized by the Home Health Agency and confirmed as competent by the Clinical Manager.
PREPARATION AND EDUCATION
- Prepares necessary equipment and materials for treatments while observing aseptic techniques when required.
Assists patients in acquiring effective self-care skills.
- Demonstrates proficiency in delivering patient care/services, ensuring compliance with professional standards and state nursing regulations.
- Provides relevant patient education and appropriate teaching resources.
- Reviews and updates the patient's medication profile, noting any new, altered, or discontinued medications, and informs the Registered Nurse of these changes.
- Communicates effectively with other team members regarding the patient's condition as needed.
- Participates in staff meetings, educational workshops, and committee discussions as appropriate.
- Engages in on-call duties as requested.
