About the Position

SaVida Health is a specialty medical practice providing outpatient treatment services for individuals with substance use disorder. We are seeking an experienced Clinical Care Coordinator to join our team.

Key Responsibilities

  • Exhibit high levels of customer service, compassion, and professionalism when interacting with patients.
  • Work closely with the center administrator and medical receptionist to schedule appointments and manage patient flow.
  • Participate in provider meetings and staff meetings as determined by the Medical Director.
  • Interpret test results, diagnose health issues, and provide feedback to patients.
  • Counsel patients regarding medication compliance and complete thorough patient evaluations and physical exams.
  • Document patient progress, including medication and treatment plans.
  • Communicate with patients' primary care physicians and specialists as needed.

Requirements

  • Experience working with electronic medical records (EMR) software.
  • Current license with prescriptive authority in good standing.
  • DEA Certificate.
  • Prior experience with substance use disorder treatment is preferred.

Compensation

The estimated salary for this position is $65,000-$85,000 per year, depending on experience.
