Job Title: LPN In Home Care

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in providing high-quality care to patients in their homes. As an LPN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role:

The Home Care LPN is responsible for providing direct nursing care to patients in their homes, as prescribed by the physician. This includes recording clinical and progress notes, coordinating patient care activities, and working collaboratively with other healthcare professionals to ensure the highest quality of patient care.

Responsibilities:
  • Provide high-quality clinical services within the scope of practice, as defined by state laws governing the practice of nursing.
  • Document patient visits thoroughly and completely, following regulatory and payer requirements.
  • Actively participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition.
  • Ensure confidentiality of patient health information, as per agency policy.
Qualifications:
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
  • Previous home care experience preferred.
