LPN Private Duty Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Chilton and Kiel. As a Private Duty LPN, you will provide high-quality nursing care to patients in their homes, working under the supervision of a Registered Nurse.
Responsibilities- Provide direct nursing care to patients in their homes, as prescribed by physicians.
- Record clinical and progress notes, and coordinate other patient care activities to ensure the highest quality of patient care.
- Work collaboratively with the healthcare team to develop and implement patient care plans.
- Ensure confidentiality of patient health information and maintain accurate records.
- Current LPN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
- Previous home care experience preferred.
