Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Chilton and Kiel. As a Private Duty LPN, you will provide high-quality nursing care to patients in their homes, working under the supervision of a Registered Nurse. This is a part-time and full-time opportunity with weekly pay and a Loyalty Bonus.

Responsibilities
  • Provide direct nursing care to patients in their homes, as prescribed by the physician.
  • Record clinical and progress notes, and coordinate other patient care activities to provide the highest quality of patient care.
  • Work collaboratively with the healthcare team to ensure seamless care coordination.
  • Ensure confidentiality of patient health information and maintain accurate records.
Requirements
  • Current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
  • Previous home care experience preferred.
