Responsibilities:
- Provide guidance and assistance to patients in assessing medical, insurance, housing, social services, community, legal, financial, and other services.
- Support the medical case management team with completion of intakes, psychosocial assessments, development and re-evaluation of individualized client service plans, and follow up on referrals as often needed.
- Perform client advocacy and patient retention activities, treatment adherence counseling, and termination or inactivation of cases.
- Maintain documentation in the EPIC, NJDOH system (CareWare), and Ryan White Grant reporting systems (CHAMPS), ensuring accurate and prompt data entry of service deliverables.
Qualifications:
- Minimum of one year's experience working in case management, discharge planning, or other health-related fields.
- High School degree required; Associate degree or higher in Social Work, Psychology, or other healthcare-related fields preferred.
- Ryan White Case Management Certificate obtained within 6 months of hire is required.
