Hemodialysis Nurse Coordinator

Key Responsibilities:
- Develop and implement individualized treatment plans in collaboration with interdisciplinary teams.
- Coordinate patient care services, including scheduling, transportation, and laboratory testing.
- Collaborate with healthcare professionals to ensure seamless transitions and optimal care.
- Participate in quality improvement initiatives and contribute to continuous learning and professional growth.
- Provide education and support to patients and their families to promote self-care and independence.
Requirements:
- Current RN licensure in good standing.
- Minimum 9 months of experience as a Registered Nurse.
- Chronic/acute hemodialysis experience preferred.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
Salary: $45.00 - $56.50 per hour
