Salary

\

The salary for this role is approximately $58,300 to $114,300 annually, based on local labor markets, education, work experience, certifications, and other factors. We offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contribution.

Job Description

\

We are seeking a skilled Registered Nurse to join our team as a Care Coordinator. In this role, you will be responsible for coordinating care services for our members, working closely with primary care teams and other healthcare professionals to ensure seamless care delivery. Your expertise will play a critical role in helping us meet our mission of improving health outcomes and enhancing the patient experience.

Your key responsibilities will include:

\
  • Establishing and maintaining communication and trusting relationships with members, family caregivers, and healthcare providers\
  • Coordinating benefits and services, including utilization review, pharmacy, and durable medical equipment\
  • Assisting members in achieving progress toward goals, adjusting individualized care plans as needed\
  • Performing administrative tasks and providing clinical support to primary care teams\
  • Participating in program effectiveness analysis and planning, identifying opportunities for improvement and implementing changes\
  • Collecting data and entering it into electronic medical records, reviewing results with healthcare providers\
  • Maintaining confidentiality of member information and adhering to state and federal regulations\
  • Traveling locally (approximately 75% of the time) to visit members in patient settings and home environments

Required Skills and Qualifications

\

To succeed in this role, you will need:

\
  • A current, unrestricted Registered Nurse license in Washington\
  • At least 3 years of RN experience, preferably in post-acute care or case management\
  • Excellent communication and customer service skills, with the ability to navigate Microsoft Office and electronic medical records\
  • A valid driver's license and willingness to travel locally
