Responsibilities

The Advanced Palliative Care Nurse Specialist will be responsible for providing end-of-life care and hospice services to patients in their homes or community settings. This role involves delivering intermittent skilled nursing services and directing, supervising, and instructing hospice aide staff in the provision of personal patient care.

Key Responsibilities:

  • Assume primary responsibility for the patient/family caseload, assessing, planning, implementing, and evaluating care needs during on-call hours in collaboration with the triage team
  • Report On-Call activities to clinical leadership and Branch RN Case Managers at the end of the call shift
  • Maintain and complete appropriate care coordination notes for visit notes, and other pertinent clinical information
  • Assist, provide, or arrange for delivery of medications and supplies needed after business hours, weekends, and/or holidays
  • Attend interdisciplinary team meetings during on-call periods for full team discussion, consultation, and evaluation of patient/family needs
  • Under the physician's order, admit patients eligible for hospice services in collaboration with the triage team
  • Report patient status and need for other disciplines to clinical leadership, attending physician, and hospice physician
  • Develop patient care plans that specifically address identified patient problems, nursing problems, and goals mutually agreed upon with patients during on-call hours, weekends, and/or holiday
  • Ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Qualifications and Requirements

To be successful in this role, you will need to possess the following qualifications and skills:

  • Current licensure in the state of practice or compliance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
  • Valid driver's license and automobile in good working condition with proof of auto insurance
  • Two years recent experience as a professional nurse
  • Ability to meet certain physical demands, including standing, bending, lifting, stooping, or performing other work requiring light physical exertion (up to 30 pounds) on a continuous basis (over 50% of time); or moderate physical exertion (30 to 50 pounds) on a frequent basis (16 to 50% of time); or heavy physical exertion on an occasional basis (up to 15% of time)
  • Ability to work independently and make sound nursing judgments
  • Capacity to assess patient needs and formulate individualized patient care plans to meet those needs
  • Knowledge of physical, psychosocial, and spiritual needs of terminally ill patients and their caregivers
  • Effective written and verbal communication skills
Preferred Qualifications and Experience

In addition to the required qualifications, the following preferences are desirable:

  • Two years' experience within hospice care
  • Certification in Hospice and Palliative Nursing
  • Experience with Electronic Medical Records
Compensation and Benefits

We offer a competitive salary package based on your skill set, experience, qualifications, and other job-related considerations. The estimated annual salary for this position is approximately $63,000 - $80,000, depending on location and qualifications.
