Job Title: Hospice and Palliative Nurse PractitionerAbout the Job

We are seeking a skilled Nurse Practitioner to join our team at AdvicaCare. As a key member of our hospice and palliative care program, you will provide compassionate care to patients and their families in their homes.

Key Responsibilities:
  • Assessments and Evaluations: Conduct routine and urgent assessments to evaluate patient needs and develop effective care plans.
  • Prescribing Medications: Order, perform, and interpret laboratory and radiology tests within your scope of practice, and prescribe medications as needed.
  • Care Coordination: Collaborate with the interdisciplinary team to ensure seamless care coordination for patients and their families.
  • Patient Education: Educate patients and caregivers on disease processes, symptom control, treatment options, prognosis, and advance care planning.
Requirements:
  • Certifications and Licensure: Current unrestricted Nurse Practitioner certification in Michigan, DEA Certification, and ability to maintain confidentiality.
  • Experience: Minimum one-year experience in a Medicare-certified home health/hospice agency preferred.
  • Skill Set: Strong interpersonal skills, sound judgment, effective organizational and prioritization skills, attention to detail, tact, dependability, emotional intelligence, and ability to work effectively in a team environment.
