Ostomy Care Specialist

About the RoleWe're seeking a skilled RN to join our team as an Ostomy Care Specialist. In this role, you'll provide high-quality, patient-centered care to individuals requiring ostomy care services in their homes. Your expertise will enable patients to maintain their independence and live fulfilling lives.
Responsibilities- Provide Ostomy care visits to patients in their homes, promoting independence and self-management of their condition.
- Collaborate with interdisciplinary healthcare teams to develop and implement comprehensive care plans tailored to each patient's unique needs.
- Conduct regular assessments to identify potential complications and provide timely interventions to prevent readmission or adverse outcomes.
- Current Michigan Registered Nurse (RN) licensure.
- A minimum of one year of experience in home health nursing, preferably in Ostomy care.
- Graduation from an accredited WOC nursing education program.
- Prior work experience as a Wound/Ostomy consult RN working with interdisciplinary healthcare team members.
- Demonstrated knowledge and skills to provide competent, compassionate care to all ages of the population.
- Strong communication skills, both verbal and written.
- Updated Tuberculin (TB) test and current CPR certification.
- Clear Criminal History/Drug Screen.
