Youth Support Housing Coordinator

The Youth Support Housing Coordinator (YSHC) is responsible for delivering comprehensive case management services to individuals facing homelessness within the Youth Homelessness Demonstration Project (YHDP). This role is primarily field-based, emphasizing client-centered interactions and support.
Key Responsibilities:
- Manage a caseload of up to twenty-two young individuals referred through the Coordinated Entry System, typically averaging around 15.
- Collaborate with participants to address barriers that may hinder their access to stable housing.
- Implement core principles of Housing First, Trauma-Informed Care, and Motivational Interviewing.
- Conduct necessary assessments with youth as part of the YHDP program intake process.
- Provide flexible, strengths-based case management tailored to the needs of participants.
- Assist youth in identifying their personal goals and aspirations.
- Develop housing stability plans aligned with the identified goals for both youth and their families.
- Conduct bi-monthly home visits and maintain weekly communication to monitor progress and coordinate services.
- Facilitate the housing placement process, acting as a liaison between youth, landlords, and service providers.
- Support participants during their move-in process, ensuring they have essential household items and understand their tenant responsibilities.
- Connect participants with resources that promote their safety, well-being, and long-term stability.
- Engage with community resources for housing, employment, and treatment services, making referrals as necessary.
- Ensure compliance with data entry requirements in the Homeless Management Information System (HMIS), including timely case notes and participant updates.
- Maintain accurate and organized case files, both physically and electronically.
- Participate in service coordination meetings and training programs.
- Exhibit effective communication skills to foster relationships with peers and participants.
- Travel as needed to transport clients and attend relevant meetings.
- Adhere to the ethical standards and policies of IMPOWER, Inc.
- Complete all required training within designated timeframes.
- Fulfill additional tasks as assigned to support IMPOWER's mission and values.
Qualifications:
- Associate's Degree or equivalent college credits in a social services field; Bachelor's Degree preferred.
- Minimum of two years of documented experience in case management, particularly with youth.
- Successful completion of background screenings as required.
- Valid driver's license and auto insurance, with a clean driving record.
- Negative results from a multi-panel drug screening.
