About the Role

We are seeking a highly skilled Clinical Pharmacy Specialist to join our team in the Hematology/Oncology department. As a key member of our interdisciplinary care team, you will be responsible for providing expert clinical pharmacy services to patients with hematological and oncological conditions.

Key Responsibilities:

  • Develop and implement evidence-based order sets for hematology and oncology patients
  • Collaborate with healthcare teams to optimize medication therapy and improve patient outcomes
  • Provide education and training to healthcare staff on hematology and oncology-related topics
  • Participate in quality improvement initiatives to enhance patient care and safety

Requirements:

  • PharmD degree from an accredited College of Pharmacy
  • ASHP-accredited PGY1 Pharmacy Practice Residency or equivalent experience
  • ASHP-accredited PGY2 Oncology Pharmacy Residency or equivalent experience
  • Board Certified Oncology Pharmacist (BCOP) certification
  • Current licensure in the State of Delaware
